Repatriation of human remains

Required documents:

  • Two original copies of the death certificate;
  • Copy of the birth certificate of the deceased
    • (The copy does not need to be notarized);
  • Itinerary;
  • Letter from the mortuary describing the casket/urn
    • (if casket, it must be hermetically sealed);
  • Permit for disposition of human remains issued by the State's Department of Health, proving that the body is in condition to travel and that there is no risk of disease;
  • Notarized letter from the family of the deceased stating the person/institution responsible for the remains at destination;
  • Cover letter explaining the service you require and your contacts (email and cellphone number);
  • Self-addressed postmarked envelope;
  • Check payable to "Portuguese Consulate".

The United States is party to the Convention Abolishing the Requirement of Legalization for Foreign Public Documents, therefore in order to be valid in Portugal, public USA documents must be certified with the Apostille of the Secretary of State of the State where the document is issued.

  • IMPORTANT: Make sure you send all of the required documents. Incomplete applications will not be accepted and will be returned.
  • Please send your request at least five work days in advance.
